I suggest doing an overall prompt with 45-50%, yours is 50% which is fine. Your refiner might be better at 0%-5%. I start off my images with 45% overall prompt and 0% refiner. Then increase it by 5% in one or both categories

Just trying to help because the misplaced limbs and digits were an issue for me until I changed the ratio and used juggernaut.
