with some models (DreamShaper XL Lighting in particular), it tends to stretch limbs and neck. So I add "Long neck, elongated neck, rubber limbs) in the negative prompt, at -3. You may also have had the refiner set at some non-zero value, which in my opinion doesn't do well with skin surface, it adds all sorts of weird lumps.
